Caregiver Assistance Program (CAP)

Designed to supplement the assistance an individual receives from his/her natural support network of family, friends, and neighbors as well as from community agencies and volunteer groups. By applying select services in a uniquely designed care package that builds on this caregiving capacity, CAP not only provides services but strengthens this vital foundation of natural support. Provides specific services with a per-month cap. Program may require a cost share from the participant. CAP recipients also receive Medicaid State plan services

Eligibility Requirements

Must be:

• Age 65 or over and be eligible for Medicare or have other health insurance that includes hospital and physician coverage.
• Individual under age 65 must be determined disabled by the Social Security Administration.

Income Guideline

Maximum of $1,911 /month individual

Allowable Assets

$2,000

Married couples apply for benefit screening

A spouse’s income is not considered in determination of eligibility, however, the spouse’s resources are.
